Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.
Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Payment is expected at the time treatment is rendered, unless special arrangements have been made prior to your appointment. We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard, American Express and Discover credit cards. If you have any questions about financing or payment, ask us! We will thoroughly explain your choices and work hard to accommodate your needs.
We participate with many insurance carriers. If your employer’s insurance covers treatment, you will receive the benefit of reduced personal costs. Policies vary, we will review your employer’s insurance plan with you to determine the appropriate course of action. Once treatment has started, as a courtesy to you, we will file your claims.
